Center Valley logistics inc, Inc. The rate above shall remain ineffect until cancelled by either party giving written
notice to the other. This confirmation is subject to the terms of the master broker-carrieragreement. Payment will
not be made unless a current signed broker-carrier agreement is on file. Carrier's insurance must be written by
aninsurance company having a Best's rating of B+ VII or better and must be authorized to do business under the
laws of the state or province inwhich the carrier provides transportation as specified in this load confirmation. All
accessorial fees must be reported within 24 hours ofdelivery, and proper documentation must be faxed/emailed in
for reimbursement. If load is "double-brokered", agreement is void and yourinvoice will not be paid. Freight
charges submitted that deviate from this rate confirmation will not be honored unless there is a writtenamendment
to the contract acknowledged by both parties. Rate confirmation must be signed and returned to Center Valley
logistics inc emailimmediately. All efforts will be made to pay the carrier within 30 days of receipt of clean BOLs
and carrier invoice. This does not guarantee 30day terms. Your drivers are required to email us the BOLS and
lumper receipts immediately after delivery of the load. Failure to do this within48 hours will result in a fine of $100
or more per day until BOLS are received. Lost BOLS will result in a $500.00 or more document recovery fee.
Carrier/Driver is responsible for lumper fee at delivery location. Center Valley logistics inc will reimburse lumper
fee with receipt turned in with bills
If Carrier misses Pick up or delivery appointment. We will charge the carrier whatever we have to pay to reschedule
