Dear Sir/Madam,

I am writing this letter regarding my flight to Milan this afternoon.

I bought a ticket to Milan, flight # SU 602, departure time May 16th 13:00. I was at the registration desk 3 hours in advance however, clerk at the desk made a mistake with my boarding pass. Unfortunately, they found out about the mistake right before boarding on the plane. There was no way to rectify this negligence at the spot, so I had to miss my flight.

I was going to Milan to spend my weekends. I had booked and paid in advance for my hotel and two excursions around the city. Airline representatives at the airport informed me about absence of vacant seats in any other flights for today and tomorrow.

Taking into account that this inconvenience happened because of your employee, I request full refund for my air ticket and unrefundable fees for hotel stay and excursions. I am attaching the receipts as a confirmation of the payment.

Please give this matter immediate attention.
