Foyle Young Poets of the Year Award 2023
Parent or Guardian Permission Form (11-12 years)

Young people aged 11-12 need to submit a permission form from a parent or guardian in order to enter the Foyle Young Poets of the Year Award. Under GDPR, The Poetry Society is not allowed to keep the young person’s information, such as contact details, without this permission. The deadline for entries and permission forms is 31^st July 2023.

If you have been asked to fill out this form by a school, library or youth group please return the permission form to the teacher, librarian or youth leader who requested it so they can pass it on to The Poetry Society.
Entering online: Please ensure the permission form is uploaded to the Foyle Young Poets of the Year Award website (www.foyleyoungpoets.org ) before the 11- 12 year old has entered the competition.
Entering by post: Please attach this form to the postal entry, and ensure you also complete the permission section on the postal entry form.
