ELD Provider Information Request
1. Provide the following information:
Company info
This company will be shown on FMCSA’s list of registered ELD providers. We will need a 
business name, address and contact info.
Domain for ELD Portal
This domain will point to your ELD portal for fleet managers. 
Your management portal will be 
uploaded to a subdomain such as 
portal.your
domainname.com. ​(if you don’t have a domain, please register it at

(or any other domain provider) and provide us with temporary 
credentials) 
Your Website 
This website will be shown on the FMCSA’s list of registered ELD providers.

Public Email 
This email will be shown on the FMCSA’s website. We recommend using a branded business 
email such as

2. Create a new user account at the FMCSA website using the above info and let us know when the
account is ready (

3. Provide access to your Google Play Store Developer account (we will upload the ELD app to your
account:
a. Create Google Play Store Developer account (if you don’t have one)

b. Add ​

n Admin.
Go to “All applications” in the top left corner, select “Developer account”, select “Users &
permissions” and invite a new user

 in the role of “
Admin”. Bender
should have permissions to accept "app signing terms".
4. Let us know when the above steps are completed and we will send you the ELD device registration
instructions and you will be able to complete the submission.
