Assumptions of patients – need careful studying for the several reasons. At 
the beginning of ignoring them we ignore opinion of the patient and him[5,6]. Also 
it is a good occasion to insult misunderstanding. The answer should be begun with 
a positive word and to explain the present disease or its reason – «Yes you are 
right, the child has a cold, but there are a lot of reasons for cold, besides that it was 
cold yesterday other children in your home may have had a flu or you visited 
someone» or «yes hypoglycemia brings to anger and mood changes, but we should 
know precisely the glucose level in your blood and to lower a dose of insulin you 
should measure it 2-3 times a day».

On the other hand, nobody likes being wrong and it is true in even with 
patients. From words «No, you do not have anemia, your hemoglobin is 120 grams 
per liter», «there is no possibility of help of cranberry juice at an asthma of the 
child» both of the patient and the doctor will not win. It is possible to tell the same 
words differently, with positive words – «yes you can rationally eat even at normal 
level of the hemoglobin, adequate quantity of energy, fibers, fats, carbohydrates 
and micro elements important for good state of health. It is not obligatory to you to 
have iron medicines.» Or «cranberry juice is also a good idea, but it cannot replace 
medicines».
Explanation – is also an important part of consultation of the patient, 
especially on difficult patients[7,8]. By explaining to the patient, you take control 
over consultation onto your hands. It is more important at consultation of difficult 
patients who like dominating. It is necessary to explain calm, in lower tone and it 
forces the patient calm down. To the patient who wants to receive the answer to a 
question «why should it take so long to wait on turn?» or «why do all doctors 
diagnose the same diagnosis and prescribe identical medicines?» you can explain 
an etiology and pathogenesis of the disease. This explanation will probably cause 
less indignation.
In the explanation doctors should use a sheet of paper for notes of keywords 
and drawing and in the end of consultation he can give this paper to the patient. 
Surgeons use drawings for an explanation of forthcoming surgical operation. It is 
possible to draw a daily diet of the child, a calendar of vaccinations, a curve of 
growth of the child and many other things. 
As it has been shown, there are many kinds of consultations of difficult 
patients on practice and all of them are suitable on the certain cases. But during the 
same time doctors should give more concrete information concerning consultation, 
for example, how to collect the analysis. 
Students were given the list of questions to get history of diseases. Three of these 
questions are open and gives more freedom to the patient to express the thoughts 
and fears.
