INTRODUCTION (annotation of the doctoral (DSc) dissertation)
The aim of the research work: to establish the frequency of occurrence of
chronic lower respiratory tract diseases in children, to identify their clinical,
biochemical,
immunological, functional, psychological characteristics and to
develop a complex of therapeutic measures aimed at preventing the progression of
the disease.
The object of the research work:
218 sick children with chronic bronchitis
and bronchiectasis who were hospitalized at the pulmonology department of the
Republican Specialized Scientific and Practical Medical Center of Pediatrics.
The scientific novelty of the research is as follows:
proved the role of significant prognostic risk
factors - perinatal conditions,
passive smoking, premorbid background, associated and previous diseases in the
formation and development of chronic bronchitis and bronchiectasis in children
(RR>1,0; p<0,001);
The pathogenetic significance of the pro- and antioxidant
status of patients
with chronic lower respiratory tract diseases, expressed as an increase in the
content of MDA, DC, decreased levels of SOD and catalase,
is most pronounced in
children with bronchiectasis, allowing them to be used as an additional diagnostic
criterion for assessing the activity of the inflammatory process;
Informative diagnostic markers of chronic lower respiratory tract diseases in
children were identified, taking into account their vegetative reactivity, determined
by a 1,2-fold increase in pro-and anti-inflammatory cytokines IL-1β, IL-4, IL-6,
IL-8, and a significant positive the correlation between the indicators of
prooxidant, antioxidant systems and immune status (r=+0,53-0,59 and r=+0,52-
0,61 p<0,001);
determined the role of the autonomic reactivity of the body, characterized by
the predominance of hypersympathicotonic initial autonomic tone in combination
with hypersympathicotonic and asympathicotonic vegetative reactivity, compli-
cating
the course of the disease;
for the first time, pathogenetically substantiated differentiated methods of
treatment of chronic bronchitis and bronchiectasis in children have been improved,
taking into account their vegetative reactivity.
