Academic Misconduct Allegation, Record of Meeting Form and checklists

Instructions for filling out this form:

1. 
   The first section must be filled out by the Module Leader/off campus tutor in all cases
   
2. 
   If this is a Stage 1 case (minor penalty) please additionally fill out Section A
   
3. 
   If this is a Stage 2 case (more serious offence) please additionally fill out Section B
   
4. 
   Submit this form to [e-mail], including all evidence (such as Turnitin reports etc.) within four weeks of the marking period or within two days of the student interview, whichever is the sooner
