2022 Shawnee, USA October 1 - m. uvulae; 2 - m. palatopharyngeus; 3 - m. tensor veli palatini; 4 - m. levator veli palatini; 5 - m. constrictor pharyngis superior. Pathological attachment m. levator veli palatini (MLVP) - the muscles that raise the soft palate with through and hidden (submucosae) cleft palate were described in detail by Bluestone C.D. et al., Heidsieck D.S. et al., Logjes R.J. et al. and Park T.S. et al. (2016); Villa P.M. et al., Martin A.K. et al., Logjes R.J. et al., Alper C.M. et al. and Ikhwan M. et al. (2017); Bashiruddin J. et al. (2018). Muscles that lift the soft palate m. levator veli palatini are normally attached to the Eustachian tube and the mouth of the carotid canal, but in pathology they are fixed to the posterior edge of the hard palate and in the structures of the tendon of the tensor muscle m. tensor veli palatini - instead of attachment in the area of the midline of the soft palate (Fig.3). Fig 3. Schematic representation of the muscle of the eustachian tube (Vila P. et all. 2017). One of the forms of an isolated cleft palate is a congenital latent form (submucosae), which manifests itself in such a way that it seems to be a small pathology in its form and appearance, however, speech disorders present in this form, no less than in others, lead to the need thorough examination and treatment. The latent form of the cleft is one of the forms of palate pathology in which the main cause of speech disorders can be anatomical and functional disorders ISBN 978-1-64945-234-4 DOI https://doi.org/10.5281/zenodo.7509018 20 CUTTING EDGE-SCIENCE 2022 Shawnee, USA October associated with damage to the speech-reproducing structures of the articulatory apparatus. In some cases, foreign authors refer to the hidden cleft as a submucosal type and call it submucosal (submucousае cleft palate) (Bluestone C.D., Otteson T.D. 2016) cleft palate. In Russia, it is customary to call it a hidden form of cleft palate. In our opinion, both are correct, and we also refer it to a congenital cleft palate of a latent form. In some cases, foreign authors call the latent cleft in relation to the submucosal wa species its submucosal (Bluestone C.D., Otteson T.D. 2016) cleft palate. In Uzbekistan, it is customary to call it a hidden form of cleft palate. In our opinion, both are correct, and we also refer it to congenital cleft palate of a latent form. It should be noted that the establishment of a clinical diagnosis in this case is the competence of not only the surgeon, but also the speech therapist, pediatrician. - the middle cut on the bone base of the solid sky, as the rule of the triangular form; - lack of muscle mass, soft tissue, and non-porous mucous membrane; - uvulae. In this case, the diagnosis of congenital latent cleft palate can be established in the presence of retraction in the region of the tongue, thinner mucosa along the midline of the soft palate, and muscular bulges along the edges from the midline. In more difficult cases of diagnosing congenital occult cleft palate, we note that the muscles of the soft palate are split, the nasal and oral mucosa are inseparably located, the area of the soft palate looks whitish or partially translucent. A bone defect in the hard palate along the midline can have a variety of shapes and lengths: triangular acute at the apex or horseshoe-shaped, sometimes reaching the incisive foramen. With a preserved pharyngeal reflex, congenital latent cleft palate tends to "visibly" increase the defect on the hard palate. The bulge of the muscles of the soft palate, in its central part, smoothes out when closed. A light source placed in the nasopharynx, behind the soft palate, shows the true defect (diaphanoscopy). Palpation confirms the actual size of the hard palate defect. It should be pointed out that an impression on the uvula (uvulae) is not a reliable sign of congenital latent cleft palate, since the ISBN 978-1-64945-234-4 DOI https://doi.org/10.5281/zenodo.7509018 21 Download 0.9 Mb.
