a. Who: Chosen students
b. What: Pre-test survey examination
c. When: On the assigned date. Starts at 4:30 pm
d. Where: Assigned in 3rd floor of Liceo de Cagayan University, RNP Campus
e. Why: To identify the selected students’ level of math skills
f. How: To answer the given test questionnaire and following the instructions given
by the researchers.
II.
Things that the selected students need to bring are on the following:

Consent Form
After writing their name in the attendance paper, the researchers will collect the
consent form of the students to know if there are one or more students who are not
there in the said activity

Pencil
This will serve as the students’ tool in answering the multiple-choice type of test
questionnaire.

Eraser (not necessary)
This item is not necessary, but the participants can bring this if their pencil doesn’t
have an eraser.

Sharpener (not necessary)
This item is also not necessary, but the students can use this incase the tip of their
pencil is broken.

Piece of bond paper/any blank paper

Participants may use bond paper or any type of blank paper they may use to solve
the test questionnaire questions.

After the survey, the researchers will clean the vacant classroom and arrange
the chairs in its original position. Then, gathered the data through respondents’
name, grade & section, date, name of their math teachers and grade in mathematics
subject. Moreover, the researchers will check, interpret, and tally the scores of the
test questionnaire of the selected grade 9 students and rate their scores afterward.
