Simulation of spleen damage for reproduction of active lethal bleeding

Relevance: The spleen parenchyma does not contract with trauma, so bleeding 
can be intense and prolonged. The main cause of bleeding is considered to be 
abundant vasculature and congestion with blood, as well as poor contractility of the 
spleen parenchyma, leading to prolonged bleeding even with minor injuries. 
Currently, there are no adequate models that could recreate splenic injury in 
experimental animals with bleeding similar to that in humans. The new model allows 
evaluating the effectiveness of various methods of hemostasis in spleen injury, which 
can be used in clinical practice. 
The goal is to develop a new method that allows to achieve active bleeding that 
recurs and leads to death without therapeutic measures. 
Materials and methods: To solve the tasks set, a method is proposed for 
experimental modeling of damage to the spleen in rats with mixed bleeding by 
inflicting a wound with a scalpel, characterized in that a linear wound is applied along 
the outer surface of the spleen to the capsule of the inner edge of the spleen for a 
length of up to 2 cm. In this case, active mixed bleeding is achieved, which has no 
tendency to self-stop. Known methods of forming a model of damage to the 
parenchymal organ by applying a superficial wound using an abrasive tool, however, 
in such cases, capillary bleeding is modeled. With abrasive action, the capsule of the 
parenchymal organ is damaged, it should be noted that in rats, after damage to the 
spleen capsule, bleeding can stop spontaneously, while in humans it is prolonged. 
The main reason is that capillary bleeding occurs in rats, while mixed bleeding occurs 
in humans with damage to the spleen. Therefore, it is incorrect and unreliable to use 
the abrasive method of damaging a parenchymal organ in an experimental animal 
with its subsequent surgical treatment and extrapolation of the results obtained to 
possible damage in a human organ.
