Loads sealed by the shipper must remain sealed until an authorized person at shipper or receiver breaks it. If the
seal is broken without authorization the carrier may be billed the FULL value of the load plus any additional
expenses incurred.
$25.00 will be deducted off your invoice for EACH EFS issued.
Center Valley logistics inc reserves the right to offset any claims or fees with other pending invoices.
Carrier agrees that driver has enough available HOURS OF SERVICE to pick up and drop off this load within the
time frame indicated without violating the current FMCSA HOURS OF SERVICE regulations

Carrier shall endeavor to maintain a satisfactory U.S. DOT safety rating but under no circumstances is carrier
allowed to provide services under this contract if their safety rating falls to "unsatisfactory."
Carrier agrees that they are in compliance with Federal, State and Local safety laws and regulations.
All accessorial charges must be approved in writing by Center Valley logistics inc or they will not be honored.
All detention must be signed for on bills of lading by either the shipper or consignee in order to be paid
Detention Policy depends on the customer. If customer pays or not
All refrigerated loads must be on CONTINUOUS cycle through the entire trip unless instructed by Center Valley
logistics inc. If unit is not set on continuous carrier assumes all risk of product damage. Failure to do upon request
will result in a claim
Before loading, check to make sure the reefer unit is working properly and pre-cool trailer to temperature indicated
